Just started a new #aviation #book, "Pilot Error: The Human Factors" by Ronald and Leslie Hurst. Ought to be an interesting read. Here's a pull quote from the Introduction: "The Human Factors area today, more than ever, must ensure that adequate attention is given to the failure mode in the human equation." The book is copyright 1982, and brings together many human factors experts to dissect several big airliner crashes, including the Tenerife 747 crash that tragically had 583 fatalities.
