Sorry it took so long to resolve. :blobcatohno: All admins were busy with other things and notifications being broken also meant we didn't get the mentions pinging us about the issue.
Monitoring didn't trigger because all API endpoints and workers were working fine. We'll see if we can maybe check worker logs for unusually high numbers of exceptions automatically.
