Back in June 2008, staff in a lab (next to lab where I had worked & across the back yard from my residence in the 80s) found a cracked vial. Some of the release ended up in a lab industrial sink that drained to public sewer when a staff member used it to wash their hands. This was a (thankfully very small) Pu spill to public sewer.
